The CHSP provides entry-level support for older people who need some help to stay at home. Service providers work with them to maintain their independence. Support can include help with daily tasks, home modifications, transport, social support and nursing care.

What services do HACC provide?

HACC (Home and Community Care)

  • In-home nursing care.
  • Allied health services (physiotherapy, podiatry, speech pathology etc.)
  • Domestic assistance (cleaning, shopping, washing etc.)
  • Personal care (bathing, grooming, dressing, eating, meal preparation etc.)
  • Social support.
  • Gardening and home maintenance.
  • Home modifications.

Who is eligible for Chsp funding?

To be eligible for this program, you must be aged: 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or. 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless, or at risk of being homeless.

Is Chsp income tested?

CHSP is not means tested. The hourly fees are very affordable. If you do need more assistance you do not automatically switch over to Home Care. The ACAT assessor will visit you at home or in hospital to review your health and living arrangements.
